Medical Certificate Apostille and Attestation for Visas, Jobs and Study
A medical certificate apostille confirms to a foreign office that your Indian fitness or health certificate carries a genuine, verified signature. In India these certificates come from a registered medical practitioner, a government hospital or a civil surgeon, and the doctor's registration sits with a State Medical Council under the National Medical Commission. INFINITYGRO takes the certificate through notary, SDM and the Ministry of External Affairs, and adds embassy legalization where your destination needs it.

Can a private doctor's medical certificate be apostilled? Usually yes, if the certificate is on the doctor's letterhead, signed, stamped and shows a valid registration number. For a medical certificate apostille on the Delhi route, a notary verifies it first, then the SDM, then MEA adds the apostille. Some employers and universities abroad accept only government hospital certificates, so check the checklist before you get examined.
Everything starts with the doctor's registration
A medical certificate is only as strong as the person who signed it. Authorities in India and abroad want to know that the signature belongs to a doctor who is legally allowed to practise. That is why the registration number on the certificate matters more than the hospital logo.
Doctors in India register with a State Medical Council. The National Medical Commission (NMC), which replaced the Medical Council of India, publishes these registrations in its Indian Medical Register. Anyone can search it by name, registration number, year or council. Many clients still call it "MCI registration", and older certificates may say so.
Before you start, ask the doctor to write the registration number and council name clearly beside the signature. A missing number is the most common reason we send a medical certificate back.

Kinds of medical certificates sent for legalization
The term covers several different papers. The route is the same, but the wording each foreign office expects is not.
- Medical fitness certificate for a job offer or work visa abroad.
- Fitness certificate for admission to a foreign university or training course.
- Health declaration for a long-stay or residence visa.
- Medical leave or illness certificate needed in a foreign court, school or employer matter.
- Disability or treatment certificate from a government hospital, used for insurance or benefits overseas.
Each of these needs its own medical certificate apostille. If the receiving office gave you its own form, have the doctor fill that form. A plain letter may be rejected even after it carries an apostille.
The medical certificate apostille route through Delhi
Most personal documents from any Indian state can follow the Delhi route. It runs in three steps.
- Notary (Delhi). A notary checks the certificate and records it.
- SDM (Delhi). The Sub-Divisional Magistrate's office verifies the notary's signature.
- MEA. The CPV Division of the Ministry of External Affairs places the apostille or normal attestation stamp.
MEA checks only the signature of the officer before it. It does not judge your health, the diagnosis or the hospital's standards. Its stamp tells the foreign office that the Indian chain of signatures is genuine.
Some receiving offices ask for the state route instead. Then the certificate goes to the Home or General Administration Department of the state where it was issued, and from there to MEA. We follow whatever the receiving office's checklist says.

Government hospital or private clinic?
Both can work for the Indian side of the process. The difference shows up abroad. Some Gulf employers and certain universities ask for a certificate from a government hospital or a civil surgeon only. Others accept any registered practitioner.
Read the employer's or university's instructions before the medical test. Getting a second certificate after the first one is already apostilled costs you time and fees twice.
For Gulf work visas, the destination country often runs its own medical test after arrival or at an approved centre in India. A legalized Indian certificate may be required in addition to that test, not in place of it. Your employer will tell you which applies.
A police or school character certificate is a separate document with its own route. If your employer asks for both, see our character certificate page and we can process them together.
When an embassy step is added
Apostille countries accept the MEA apostille directly. These include the UK, USA, Canada, Australia, most of Europe, Japan, South Korea, Saudi Arabia, Oman, Bahrain and Turkey. Vietnam joins from 11 September 2026.
Other countries need MEA normal attestation and then the embassy in New Delhi.
- UAE: MEA attestation, UAE Embassy, then MOFA attestation in the UAE (done online). See our UAE attestation fees page.
- Qatar, Kuwait, Egypt, Iraq, Jordan: MEA attestation then the embassy. Our embassy attestation service covers these.
- China: India objected to China's accession to the Apostille Convention, so the embassy route applies.
- Germany: German missions do not legalize Indian documents. German authorities may ask for verification through the German Embassy instead.

Cost and time on the Delhi route
For a medical certificate apostille, our all-inclusive Delhi route fee is Rs 1,000 to Rs 1,500 per document. That covers notary, SDM, the MEA government fee of Rs 50 and our service. Courier is extra, and embassy fees are extra for non-Apostille countries.
The Delhi route usually takes 2 to 3 working days once we have your documents. We confirm the exact quote in writing before you send anything. For more on how the apostille itself works, read our MEA apostille guide.
Mistakes that delay a medical certificate
- No registration number or council name near the signature.
- Name spelled differently from the passport.
- Certificate laminated. Stamps cannot be placed on lamination.
- Photocopy sent instead of the signed original. MEA does not legalize plain photocopies.
- Certificate too old for the receiving office. Many employers want one issued within a recent period, so check their limit.
- Apostille obtained for a country that needs the embassy route, or the reverse.

Medical Certificate apostille FAQs
Does MEA check whether I am medically fit?
No. MEA verifies only the signature of the officer before it in the chain. It does not review the medical findings or the doctor's opinion.
My certificate says MCI registration. Is that still valid?
The National Medical Commission replaced the Medical Council of India, but doctors' registrations with State Medical Councils continue. A certificate that quotes an older MCI or state council number is normally fine if the number is genuine and readable.
Can a medical certificate apostille be done for a UAE job?
The UAE is not on the Apostille route. Your certificate needs MEA attestation, then the UAE Embassy in New Delhi, then MOFA attestation in the UAE.
Can I get the certificate from a doctor in another state?
Yes. Personal documents from any state can usually take the Delhi route. If the receiving office asks for the state route, we send it to that state's Home or General Administration Department first.
Will an apostilled medical certificate replace the medical test abroad?
Usually not. Many countries still run their own test. The legalized certificate is a supporting document. Your employer or visa office decides which they need.
Is a digital medical report accepted?
A printed lab report alone is rarely enough. Most offices want a fitness certificate signed and stamped by the doctor. Ask the doctor to issue one based on the reports.
How many apostilles do I need if I have several certificates?
Each signed document gets its own stamp. A fitness certificate and a separate disability certificate would be two documents, so two medical certificate apostille stamps and two fees.
